7 Quick Apply Part-time Contract 1 hour ago Full Job Description Our Medical Director performs the initial in-home admission consultation for all new patients starting with our advanced illness program.
The Director is then responsible for the coordination and oversight of primary and palliative care house calls and for the quality of services rendered.
Medical care is provided to seniors living in residential homes and senior living communities.
Our program has a longitudinal care plan that treats the whole person in an integrated way, including both treatment and disease prevention.
Our holistic interdisciplinary team is coordinated by a Nurse Navigator and includes:
Nurse Practitioners, Nurses, Therapists, Social Workers, Home Health Aides, Behavioral Health Specialists and Healthcare Strategist.
Our Medical Director should have strong management experience in house calls, chronic disease management, gerontology, palliative care and dementia.
Must have mentoring skills and a depth of experience in managing diverse clinical care teams.
Requirements MD or DO with unrestricted license in Virginia 8
years practicing as a geriatric physician 2
years performing geriatric house calls Active unrestricted DEA registration Strong computer skills Job Types:
