74 an hour The Health Liaison facilitates health and mental health support and assistance for LSSNCA clients with the goal of eliminating health related barriers to self-sufficiency and community integration.
The role of the Health Liaison is to connect clients to appropriate care, while also providing health education and assistance with accessing affordable health care (such as establishing primary care or specialized care within the required time frame).
The Health Liaison also conducts outreach with local healthcare providers to increase cultural awareness of the unique needs of refugee and immigrant clients.
Essential Functions Client-Centered Services ? Ensure timely coordination of medical services in accordance with RHEO requirements; ? Act as intermediary between clients, local health department, medical providers, and resettlement agency staff to facilitate services for newly arrived refugees; ? Conduct health-related orientation(s) to newly arrived refugees as part of local cultural orientation program and provide written information on health topics to refugees as needed; ? Develop health education programs and materials ? Provide guidance to clients on state and federal health insurance programs; ? Oversee cases with significant medical needs.
Provider Relations/Outreach ? Provide trainings for local health service providers on the special medical, cultural and linguistic needs of refugees; ? Maintain up-to-date list of community health resources; ? Conduct outreach to new providers.
Office Responsibilities ? Take part in all staff meetings, development activities, and other tasks as assigned; ? Comply with agency policies, procedures and protocols; ? Timely complete reports and VNIS entries; ? Other duties as assigned.
Contract Compliance and Documentation ? Ensure compliance with both case file management and reporting requirements; ? Coordinate with State Refugee Health Coordinator on any unusual or difficult cases that occur or that may benefit from support from the Virginia Department of Health; ? Participate in quarterly conference calls facilitated by the Office of New Americans- Refugee Services and submit data reports as required.
Qualifications and Education Requirements Education ? Bachelor's degree in health or human services preferred.
Experience ? At least one year of relevant work experience Other Requirements ? Valid driver's license; ? Clean driving record; ? A personal vehicle or access to a vehicle required; ? Background check Preferred Skills ? Strong verbal and communication skills; ? Detail oriented; ? Excellent problem-solving abilities; ? Knowledge of the United States Resettlement Program ; ? Understanding of health care system/federal and state health programs ? Ability to work with people from diverse cultures and backgrounds; ? Foreign language in addition to English preferred; ? Adept in team setting as well as individual work.
